Upstream Laboratories

Symptoms are downstream. We measure upstream.

Upstream Laboratories is a physician-founded specialty reference laboratory. Our panels are selected backward from the clinical question, and every report names a driver, an intervention target, and a date to re-measure.

Start Here

Six pathways, organized by the problem

Patients do not arrive with clinical questions — they arrive with complaints. Each pathway starts from a complaint a clinician hears every week, states the question the testing is built to answer, and carries a fixed panel set. The mechanism reasoning behind the selection lives in the report, where it belongs, rather than in the ordering decision.

Pathway One

Women's Hormone Health — Cycling

“My periods are terrible” “I'm breaking out and losing hair” “PMS is running my life”

We find out why your hormones are off — not just that they are — and we check the three safety questions before anything is prescribed.

What we're answering

Is the estrogen excess from overproduction or gut reabsorption? Is it routing down the safe or the genotoxic pathway? Are the androgens adrenal or ovarian? Is progesterone genuinely low, or simply unopposed?

Pathway Two

Women's Hormone Health — Menopause & HRT

“I'm in menopause” “Should I start hormones?” “I'm on HRT and want to be sure it's safe”

Before you start or adjust hormones, we check the three things that determine whether the dose is safe — and if you're already on HRT, we make sure it's being handled safely.

What we're answering

Staging the transition. Clearing the routing gate before replacement begins rather than after. Checking whether the gut is returning what the body disposed of. Monitoring dose safety for women already on replacement.

Pathway Three

Men's Hormone Health & Performance

“I'm tired and my drive is gone” “I'm on TRT — is it working?” “My testosterone is fine but I feel terrible”

Your testosterone level is one number. What your body does with it is the whole story — and if you're on TRT, we check whether the estrogen it creates is going somewhere safe.

What we're answering

Is the problem production, bioavailability, or conversion? For men on replacement: is the dose right, is aromatization excessive, and is the estrogen that aromatization produces being routed safely?

Pathway Four

Energy & Recovery

“I'm exhausted and nobody can tell me why” “My thyroid meds aren't working” “I can't think straight”

We measure five things at once that can all cause fatigue — because testing them one at a time takes months, and they interact.

What we're answering

Where in the energy pathway is the block? Is thyroid hormone present but unconverted? Is the fatigue nutrient-driven at the enzyme level rather than the blood level? Is a toxic or inflammatory load consuming capacity?

Pathway Five

Weight & Metabolic Health

“I eat right and exercise and the weight won't move” “My cholesterol is fine but my family history scares me” “Am I pre-diabetic?”

If diet and exercise aren't working, something upstream is working against you — and we find out what it is before you waste another year.

What we're answering

Insulin resistance before glucose moves. Thyroid under-conversion, which caloric restriction itself suppresses. Particle burden independent of cholesterol content. The synthesis-versus-absorption phenotype, which determines which lipid agent will work.

Gut, Detox & Environmental

“I'm bloated all the time” “I react to everything I eat” “I think it's mold” “Nobody can figure out what's wrong”

We measure what your gut is actually doing — not just what lives there — and if there's an environmental exposure, we check whether your body can handle clearing it before we start.

What we're answering

Is the gut digesting? Is the barrier intact, measured three ways rather than one? Is there inflammation that warrants a referral? Is there a toxin burden, and can the patient clear it safely before mobilization?

Comprehensive

The Comprehensive Workup

“I've been to six specialists” “Every test comes back normal” “I want the whole picture”

For the patient whose single-system workups have each come back unremarkable — and the clinician who would rather not choose which system to investigate first.

What we're answering

Which of the named drivers is operating. The value is not finding one abnormal marker — it is revealing that several systems are being pushed by a single driver, which sequential single-system testing structurally cannot show. This is where cross-panel synthesis does the most work.

CoreAll seven panels

Pathways are ordered by a licensed clinician, who receives the full report and directs any treatment. If you are a patient, the pathway names above are a reasonable place to begin a conversation with your physician: bring the complaint, and ask which question they want answered first.

The Premise

The measurement, not the school of thought

Every clinician already reasons upstream. Cardiology moved from the event to the particle. Endocrinology moved from the symptom to the axis. Upstream reasoning is not a philosophy or a movement — it is what physicians do wherever the measurement exists and is covered.

The gap is not one of belief. It is that for a large set of clinically useful questions, the measurement has not historically been available, affordable, or reported in a form a busy clinician can act on. That is an accident of reimbursement, not a difference in medicine. Our panels exist where that gap is widest.

We take no position on the division between conventional and functional practice. Both traditions order for the same reason — they want to know why — and the measurement that answers it does not depend on the ordering clinician's training. Our language is laboratory language, and it is the same in every room.

The Approach

Four mechanisms organize the entire menu

Every result on every panel comes down to one of four things going wrong. These are not marketing categories — they are the organizing logic behind why a given analyte sits on a given panel, and they determine how each result is read.

Mechanism One

Capacity against load

A sink. Water arrives from the tap and leaves through the drain. Trouble starts when more arrives than the drain can handle — and two very different problems look identical from outside.

How it presents
Something piles up in front of a step, or runs short after it.
What resolves it
Turn down the tap or clear the drain. Telling those two apart is much of why more than one panel is run.
Mechanism Two

Routing

A fork in the road. The right amount of traffic is moving and nothing is blocked — too much of it is simply turning down the wrong road.

How it presents
Always as one thing measured against another, never as a single number in isolation. This is why so much of our reporting is ratios.
What resolves it
Change which road the traffic takes. Sending more traffic is the intuitive move and it makes a routing problem worse.
Mechanism Three

Elimination

Taking out the trash. The body wraps up what it wants rid of, tags it for disposal, and sends it out. The failure is when the trash never leaves — or is unwrapped and returned.

How it presents
The substance is present but its conjugated form is missing, or the unwrapping activity is measured directly.
What resolves it
Repair the disposal route. Suppressing production is the wrong target — production was never the problem.
Mechanism Four

Signalling

Shouting at someone who is not listening. The message is being sent and it is loud. It is simply not getting through, and shouting louder does not help.

How it presents
The messenger and the thing it controls are both abnormal in the same direction. Plenty of signal, no response.
What resolves it
Restore responsiveness at the receiver. Adding more messenger is the classic mistake.

Sequencing

Three findings change the order of treatment

Three of the four mechanisms produce a rule about what must not be done until something else is checked. Each exists because acting in the wrong order makes the patient worse. These overlays are read on every patient, whether or not the primary axis was abnormal.

Check the drain before you flush

Mobilizing stored toxins in a patient who cannot clear what gets released makes them worse. Clearance capacity is established before mobilization, not after.

Check the fork before you add traffic

Before estrogen is started or increased, the metabolic route is read. If the body is routing down the unfavourable path, adding more increases what travels it.

Check whether the trash comes back

Where gut deconjugation is returning what the body disposed of, a prescribed dose behaves as a larger dose than intended — and so does anything released from storage.

The Report

The report is the product

Everything else exists to produce it. The report performs four moves in order, and a clinician who reads page one has the answer before reaching any numbers.

01

What the specimen supports

Collection quality appears above the findings, not buried in an appendix — it establishes what the rest of the report is entitled to claim. Where an axis could not be assessed it is named as not assessable and excluded from the composite. It is never silently treated as normal.

02

Root cause

Each axis is read independently, the safety overlays are read on every patient, and the composite is formed only from axes the specimen supports. The composite names a driver — not a list of abnormalities — with downstream findings identified as downstream of it, both ends referenced on the page.

03

Intervention

Given as mechanism and intervention class, never as agent and dose. The ordering clinician prescribes. There is no per-marker supplement engine, because per-marker recommendations contradict each other arithmetically. The plan is sequenced: treat the driver, re-measure, and let downstream findings move on their own.

04

Re-measurement

Every finding carries what to repeat, when, and under what collection conditions. Intervals are set by how fast the marker actually moves — inflammatory markers at eight to twelve weeks, the permeability module at twelve to sixteen, because earlier repetition produces noise rather than information.

Two interval bands, always togetherThe outer band is the established reference interval; the inner is the interpretive range. A reader can always see which one a flag came from.
Provenance on every intervalEstablished, verified in our own population, or interpretive — labelled, never merged.
Status in words as well as colourNever encoded by colour alone, so the report survives monochrome printing and remains legible to colour-blind readers.
Failure states render distinctlyBelow quantitation, unquantifiable ratio component, not ordered, and not reportable each look different. None renders as a blank.
Contradiction release gateNo report leaves the laboratory that contradicts itself. This is a hard gate, and the requirement the report specification was built around.
Length is a decision, not a byproductPage one is collection quality, the driver in one sentence, the findings, and what to do. Everything after supports it.

Exclusions

What we deliberately do not offer

A laboratory that will not sell something has told you what its results mean. Each exclusion below is a deliberate decision, and we would rather explain it than quietly carry the assay.

IgG food sensitivity arrays

No validated clinical mapping between the antibody and the symptom. We identify the mechanism instead.

Urinary neurotransmitters

Urinary catecholamines are overwhelmingly renal in origin and do not report on central activity.

Per-marker supplement engines

Two markers can each generate guidance that undermines the other. The ordering clinician prescribes.

Microbiome diversity indices

A species census does not tell a clinician what to do. We report function — enzyme activity, metabolites, and sufficiency.

Menu at Launch

Panels selected backward from the question

Each panel exists to answer a specific set of questions, and the reason a given analyte is on it traces back to one of the four mechanisms. Routine clinical chemistry runs alongside the specialty menu — one requisition, one collection, one portal.

Hormone metabolism & clearance

Three matrices — serum, saliva, and timed urine — covering production, metabolic routing, clearance, and the eleven-oxygenated androgens.

Thyroid conversion

Reference method free hormones, the conversion and diversion picture, autoantibodies, and the cofactors the conversion enzymes depend on.

Cardiometabolic

Particle burden independent of cholesterol content, the synthesis-versus-absorption phenotype, inherited lipoprotein risk, and insulin signalling.

Micronutrients

Status measures paired with functional markers, so a deficiency at the enzyme is visible even when the circulating level reads normal.

Organic acids

Energy production, neurotransmitter turnover and diversion, and cofactor sufficiency read at the point the pathway actually uses them.

Gut function

Digestive sufficiency, barrier integrity by concordant methods, deconjugation activity, fermentation products, and the inflammation referral gate.

Toxins

Burden separated from clearance capacity, because the two call for opposite sequencing and look alike on a single measurement.

Routine clinical chemistry

Metabolic panel, lipids, hepatic and renal function, electrolytes, CBC. Several mechanisms are read here — clearance capacity, signalling, synthetic reserve.

The Laboratory

Mass spectrometry as the default, not the upgrade

Our platform is built on instruments that cost a great deal to acquire and very little to run, and that are not tied to a single supplier's reagents. Once a method is established, adding an analyte to it costs comparatively little — which is why our panels can be deep without being padded.

LC-MS/MS

Liquid chromatography tandem mass spectrometry for steroid metabolites, free hormones by reference method, and conjugated species.

GC-MS

Gas chromatography mass spectrometry for organic acids and volatile metabolite profiling.

ICP-MS

Inductively coupled plasma mass spectrometry for elemental analysis — essential minerals and toxic elements.

Panel composition follows from this. Markers that did not change what a clinician does were removed rather than retained for the brochure — which both reduces cost and improves reliability, since a large share of the contradictions seen in long reports come from markers carried for breadth and interpreted out of context.
